We will post pharmaceutical agents in this category.  I would like this to be information only, so if you would, post questions and comments in the General category.  Thanks.

Pharmaceuticals may be the only answer.  I was told by a PhD in herbal therapies that drugs do not have to present in large quanties unlike the herbs.

There are some issues that should be taken into consideration.

1/  With common protozoa parasites such as Giardia, there have been adequate clinical studies to establish a working protocol.

2/  Since medicine has failed to recognize our species, we have no case studies to work from.

3/  The best models I can find is the blastocystis treatment protocols.  Badbugs.org is the best resource for this.

4/  Some good things about using the blastocystis model is that in some examples (microscope) that these appear to have some characteristics as our PPTU and appear to have many of the resistant properties as PPTU.

5/  The bad thing about the blastocystis information is that there are multiple reports of using different combinations, in other words no single therapy or single combination therapy appears to work.   For instance one person reports that the standard triple therapy of Secnidazole/Nitazoxanide/Furoate works while others report success using other combinations.

6/  I think we should stay within the boundaries of the safer drugs and not try the more risky drugs until the safer ones have been tried.

*We do not know if all people who have been diagnosed with PPTU and dealing with the same parasite.  The only way we know this is to have images made.

7/   We could share our drug combinations and narrow down thru the process of elimination  then if success if found we can repeat with other people.

Most popular drugs are Flagyl (Secnidazole/Tindazole), Nitazoxanide (Alinia), Iodoquinol, Septrin or Septra which is a dual antibiotic of Trimethoprim and Sulfamethoxazole, Rifaximin, Albendazole and possibly Amoxicillin.

  Lumicidal agents : Dioloxanide Furoate, Furoxone, Iodoquinol

I am putting paromomycin in the moderate class (not restricted, not recommended until I get more info, I thought I read some possible problems with it previously but I have not seen any serious precautions now.

Restricted agents (possible dangerous side effects) include Emetine (heart failure), some of the older quinol drugs.

FLAGYL

Flagyl is perhaps the most popular of the anti protozoa drugs, it has been known as the Gold Standard for treatment of protozoa infections, however reports among the badbugs.org site has show little to no effectivness in the treatment of blastocystis.

Some report that it has caused side-effects.   New compounds of Flagyl (Metrondiazole) have been made and reports show they have fewer side-effects.   These are known as Tinidazole and Secnidazole.   Secnidazole is preferred over both Tinidazole and Flagyl.

For the triple drug cocktail, Secnidazole is the lead drug and is combined with 2 other drugs depending on which version of the cocktail you use.

My personal experience with Flagyl::
I personally used Flagyl and Secnidazole, with the Flagyl I noticed some numbness in my hands in feet but went away.   Secnidazole caused no problems.   I took Flagyl by itself and rec'd minimal results, I felt it pushed them down but did not kill, I took quite a bit of it.   I tried the Secnidazole with Septra (see above post) with the lumicidal Dioloxanide Furoate which is the preferred triple drug therapy, obviously no results.

Detailed resource on Flagyl::

Nitazoxanide, also known by the brand names Alinia and Annita (and by Daxon, Dexidex, Kidonax, Pacovanton and Paramix in Mexico,
Nitazoxanide is a first-line choice for the treatment of illness caused by Cryptosporidium parvum or Giardia lamblia infection in immunocompetent adults and children, and is an option to be considered in the treatment of illness caused by other protozoa and/or helminths.[3]
It is used for the treatment of infectious diarrhea caused by Cryptosporidium parvum[4] and Giardia lamblia[5] in patients 1 year of age and older.

Adverse effects
Side effects are mostly gastrointestinal, and include abdominal pain (6%), vomiting (1%), headache (3%), nausea (3%)and diarrhea (4%).
In the clinical trials no serious adverse events were reported. No serious adverse events were reported in the prescribing information of the parent company.[16]

Septra (Septrin)

Septra (Septrin) is a dual anti biotic consisting of Trimethoprim and Sulfamethoxazole.  Typically it used for bacterial situations but it is now included in the revised formula of the triple drug cocktail along with Secnidazole and Dioloxanide Furoate (lumicidal).

It comes in both regular strength and DS (double strength).

for more info and side-effects see this link:
http://www.webmd.com/drugs/drug-15726-Septra+DS+Oral.aspx?drugid=15726&drugname=Septra+DS+Oral

My personal experience with it yielded no adverse side-effects.  I took it by itself and in the triple drug cocktail which included Secnidazole and Diloxanide Furoate.

LUMICIDALS (DILOXANIDE FUROATE and FUROXONE)

Dioloxanide Furoate and Furoxone are different drugs but share one thing in common in that they are lumicidal drugs.

Lumicidal drugs are anti-protozoa drugs but have more effect in the lumen of the gut.   The lumen is defined by Wikipedia as

 lumen (Lat. lÅ«men, an opening or light) (pl. lumina) in biology is the inside space of a tubular structure, such as an artery or intestine.[1] By extension, a lumen can also be the inside space of a cellular component or structure, such as the endoplasmic reticulum.

The idea behind this is that some of the protozoa will hide in the lining of the gut where traditional anti-protozoa agents cannot get to.  So these drugs help kill the protozoa hiding in the lining.   These are included in the triple drug cocktail, where Dioloxanide Furoate has less side-effects than the older Furoxone.

I took Dioloxanide Furoate with the triple drug cocktail and it did not cause any problem for me.

RIFAXIMIN::::

Rifaximin is a newer drug that was originally labelled for Travellers Diarrhea which would be considered a generic term that could cover both bacterial and protozoa infections.   It is a unique anti-biotic in that it stays within the GI tract where it can stay potent.   There have been anedoctal reports that it has been effective for blastocystis hominis, other reports state that it was taken with other drugs (see badbugs.org).

Further info and side-effects can be found here:
http://www.ncbi.nlm.nih.gov/pubmedhealth/PMH0000271

I took Rifaximin in smaller doses to evaluate my tolerance, I experienced no adverse side-effects.

ALBENDAZOLE::

Albendazole is a newer drug that is primarily used in the treatment of worms and/or flukes.   It is well-tolerated and does not carry some of the toxic side-effects like other drugs.   It is not considered a primary anti-protozoa agent but there are reports of using it as such.  I would caution against combining it with Flagyl (Metrondiazole, Secnidazole or Tinidazole), somewhere I read that people have had side-effects when used together.   I do not have a source for this at this time.

here is the link for more information:
http://en.wikipedia.org/wiki/Albendazole

My personal experience and others report no adverse side-effects in fact all reported feeling very calm while using this drug.

AMOXICILLIN::

I am posting Amoxicillin on this site in spite of it not being a first choice for protozoa.   There are reports of it being used in conjunction with Flagyl (Metrondiazole) for treatment of the protozoa Giardia.  It is an old-school drug in that it has a long track record of safety, it is penicillin.

here is the link for more information:

PAROMOMYCIN::
  I originally posted that this drug had unwanted side-effects.  In looking at the research again I am second guessing my original thoughts.   I will revisit this at a later time.   I do know that I had a difficult time locating this drug, the only place I could find was at a Central American pharmacy. (update:  I found it at goldpharma.com)

COMBINATIONS:::

Again I will model the blastocystis research concerning treatment approaches.  The best guide I can find is badbugs.org.   Blastocystis (b. hominis) is a diverse parasite that rarely responds to Flagyl even though it has been considered the 'gold standard' of protozoa drug treatments.   Part of the problem with blastocystis is that it has several forms and each form requires a different drug strategy.

I am listing some of the combinations:

1st protocol of the triple drug cocktail:

Secnidazole/Nitazoxanide/Furoxone

2nd protocol of the triple drug cocktail

2nd protcol is a revised version of the 1st having less side-effects probably from removing Furoxone as the lumicidal drug and replacing it with Diloxanide Furoate.

Secnidazole/Septrin (Septra)/Dioloxanide Furoate

Other combinations listed on badbugs:

Rifaximin with Nitazoxanide

Flagyl with Iodoquinol

Hi jyt - I've taken the oral form of Amphotericin B - and I'm afraid it did nothing for me.  Worth a try, but may not provide the results you're looking for.  Of course, if it's candida you need to eradicate, Amphotericin B is the gold standard.

Hi Woodie,

Did you try Amphotercin B powder or pills?

I asked my doctor to prescribe me Amphotercin but he didn't want. Do you have any idea where I can get it online?

Nystatin and Amphotericin B should work against Blastocystis.hominis. This bug was thought to be a yeast because of its sensitivity to some antifungals like Nystatin, miconazol.

I have candida but I'm pretty sure it's due to my parasite infection. I took a lot of stuffs against Candida untill I tried sweet annie which almost cured me and then I know that I was dealing with a parasite infection ( worm or protozoa).

I'm planning to try the combo Cipro, Clandamycin, Tinidazol for 4 weeks.

Hi Jyt - I sourced the Amphotericin tablets (Fungizone) from a pharmacy in Germany.  You will need a prescription unfortunately.  However, I believe you can also source the drug (without a script) from: http://www.pharmacyescrow.com/aboutus.aspx

In saying that, I've tried both Nystatin and Amphotericin without success. And I've never heard either being successful for b.homminis.  However, always worth a try - they will certainly help with the Candida.

Another alternative is Nitazoxanide - or a combo of drugs delivered by enema.  See my reply on another thread.
